Parents please complete the Health Insurance Information Form. Check appropriate box per their dependent's insurance coverage, sign and date.

Hobart William Smith Colleges also offers a Student Health Insurance Plan which can be purchased by contacting the Student Accounts/Business Office at 315-781-3343.

International Students are automatically enrolled in the current Student Health Insurance Plan.
