Reporting a Work-Related Injury or Illness at HWS
- This form is used to report accidents that occur on campus property.
- Complete this form if you oversee a youth program or activity sponsored by HWS. The program or activity can take place on or off campus.
STAFF AND POSITION MANAGEMENT
- This form is used to request new positions, fill vacancies, promote employees, and submit changes in salaries, status, titles, etc. Completed PAR forms must be approved by your senior division leader and submitted no later than two days prior to the next meeting date.
